The types of insurance are:
DHMOs are often the most affordable type of plan, but may also have more restrictions. DHMOs pay highly qualified dentists a fixed monthly rate for each subscriber to the DHMO, and patients are then only responsible for relatively small copay. HumanaOne is the only DHMO provider available in Alabama.
PPOs work with a wider network of dentists than DHMOs. Patients are able to choose from a list of preferred dentists who agree to provide services at a lower cost for members under the plan in exchange for greater access to patients. PPO providers in Alabama include Delta Dental, HumanaOne and Nationwide.
Fee Schedule plans list the percentage or dollar amount the provider is willing to cover for specific procedures, and the list is called the “fee schedule.” This fee schedule is determined by Usual, Customary and Reasonable fee model (UCR), which determines the cost of dental procedures based on the average cost of services from dentists within the area. They typically have lower premiums than most plans, but also come with a maximum amount of coverage per calendar year. Become aware of the fees on this option. Indemnity plans work through a “fee for service” model, and are sometimes called “traditional dental insurance” plans. Indemnity plan providers also use a fee schedule which is subject to UCR fees, and reimbursements are typically made after receiving and reviewing the bill. This may require patients to pay higher out-of-pocket costs, but they offer the greatest freedom when it comes to choosing a preferred dentist.
Discount plans can offer unlimited health care at a discounted rate in exchange for an annual or monthly fee without restrictions or waiting periods. They may be extremely attractive for patients with inadequate dental health insurance, and the savings far outweigh the cost of the discount card. Patients are usually required to pay the balance of the service not covered by the discount at the time of the visit, so familiarize yourself with each provider's rates, and check with your preferred local dentist to see how they accept payments from discount card holders.
